How it works

How does home nursing follow-up work?

  1. Medical prescription and initial contact

    Nursing follow-up is prescribed by your GP or specialist. Once I receive the prescription, I contact you to plan the visit schedule.

  2. Initial assessment at home

    On the first visit, I carry out a full assessment: vital signs, general condition, treatment compliance, living environment and specific needs.

  3. Regular monitoring according to the care plan

    At each visit, I check clinical parameters (blood pressure, blood sugar, weight, oxygen saturation…), monitor progress and adapt care as needed.

  4. Coordination and reporting

    I regularly report my observations to your doctor and alert them immediately if any warning signs appear. I can also attend multidisciplinary care meetings.

Who is it for?

Home nursing follow-up is intended for patients with chronic conditions requiring regular monitoring: type 1 or 2 diabetes (blood sugar control, insulin injection training), heart failure (weight, blood pressure and oedema monitoring), COPD and respiratory conditions.

It is also indicated for post-hospitalisation follow-up: returning home after surgery, cardiac decompensation or an acute episode. I ensure treatment compliance, monitor for complications and coordinate with the care team.

Therapeutic education is an integral part of my role: I train patients and their families in essential skills (self-monitoring, recognising warning signs, managing treatment) to promote independence and prevent hospital readmissions.

Is a prescription needed for nursing follow-up?
Yes, home nursing follow-up is a prescribed act. The prescription specifies the frequency of visits, parameters to monitor and duration of care. It is required for reimbursement by French Social Security.
How often will you visit?
Visit frequency is defined by the medical prescription and adapted to the patient's condition. It can range from once a week for light monitoring to once or twice a day for patients requiring close surveillance (insulin-dependent diabetics, recently decompensated heart failure, recent hospital discharge).
Do you communicate with my doctor?
Yes, coordination with your GP is an essential part of nursing follow-up. I regularly report my observations and contact your doctor immediately in case of any anomaly or warning sign. This ensures continuity and quality of your care.
Do you visit on weekends and public holidays?
Yes, I visit 7 days a week from 6:45 am, including Sundays and public holidays. Chronic condition monitoring cannot be interrupted at weekends — you can count on me for uninterrupted continuity of care.
